Unit 4  Admitting and Discharging the Patient

According to the doctor's diagnosis, patients requiring hospitalization should experience two admission and discharge process, the nurse responsible for patients in the process of safety and nursing care during hospitalization, or in transit, must grasp the general procedure of admission and discharge, in accordance with the overall assessment of nursing care requirements, and meet the patient's physical and mental needs, help them as soon as possible to the hospital environment, abide by the rules and regulations of hospital, actively participate and cooperate with medical and nursing work, so as to accelerate the recovery process. At the same time, nurses should be encouraged through health education and strive to improve the self-care ability of patients, guide the patients after discharge to consolidate curative effect, maintain health.

This chapter mainly introduces the admission procedures, into the ward after the initial care, grading nursing, bed preparation, comparison of various bed operation, to distinguish between the points of discharge, discharge care etc..

第四章  出入院患者护理

根据医生诊察，需要住院治疗的患者都要经历入院和出院两个过程，护士在此过程中负责病人在入院、转运或出院期间的安全和护理，必须掌握入院和出院的一般程序，按照整体护理的要求，评估并满足患者的身心需要，协助其尽快适应医院环境，遵守医院规章制度，积极参与和配合医疗护理工作，从而加速其康复过程。同时，护士还应通过鼓励和健康教育，努力提高患者自护能力，引导患者出院后继续巩固疗效，维持健康。

本章主要介绍：入院程序，入病区后的初步护理，分级护理，床单位准备，比较各种铺床术，区分其要点，出院方式，出院护理等。

1. 分级护理：是指患者在住院期间，医护人员根据患者病情的轻、重、缓、急和生活自理能力，确定并实施不同级别的护理。
