To be truly effective, a community health care system needs to (a) provide easy access to the system, (b)be flexible in responding to the care needs that individuals and families identify, (c) promote care between and among health care agencies through improved communication mechanisms, (d) provide appropriate support for family caregivers, and(e) be affordable.
